COVID-19 PRECAUTIONS FOR CTC TESTING CENTER
Check-In procedures: 
Questionnaire at the door prior to entrance into the building:
  Have you recently traveled to an area with known local spread of COVID-19?

  Have you come into close contact (within 6 feet) with someone who has a laboratory confirmed COVID-19 diagnosis in the past 14             days?
  Do you have a fever (greater than 100.4 F) OR symptoms of lower respiratory illness such as a cough, shortness of breath, difficulty           breathing or sore throat?
  Temperature WILL be taken
  If a candidate answers yes to any of the previous questions or has a temperature greater than 100.4 F they will not be allowed to               enter the building
  Candidates will enter building at individual designated time and entrance to adhere to social distancing
             Candidates will utilize door 1
   There will only be 3 candidates testing at a time in the testing center to maintain at adequate social distancing
   Work stations will be disinfected between use
    Candidates will exit the building at individual designated time and exit to adhere to social distancing
              Candidates will utilize door 1

Items allowed in the testing center: 
 A locker will be signed to your for your personal items: government issued ID(s), your car keys, snack items
 Leave ALL other items in your car including cell phones.  
 You are allowed and encouraged to wear a mask that covers your nose and mouth – Please bring one with you and have it on when         you enter the building.  
 
What you can expect:
  Staff will be wearing masks
  Items will be sanitized before/after each use
  Hand sanitizer will be available
  We will practice social distancing when we can
  We will stay home when sick 


What we expect from you:
  Wearing a mask is encouraged
  Use hand sanitizer before handling test center items
  Maintain social distancing
  Even with a mask, cover all sneezes/coughs
  Stay home and reschedule your exam if you’re feeling sick
